Tried to check the drive with disk warrior? The regular tools might not catch all possible resource corruption.

-97 = Driver Open error code (port is in use)

You don't say how repeatable it is. But might be one of the drivers in the Chooser. Disable them one by one and retest.

Yes - unless you've got a hunch on what may be the cause, use the Extensions Manager. Most Chooser items will be in the Extensions folder.

The technique is the same as any extension conflict trackdown.
Start with everything off and enable items (individually or in groups), or do the reverse - start will all on then disable items. Both strategies are valid.

Not sure about System7 but on 8.1 Conflict Catcher helped me a lot with problems like extensions misbehaving at boot time.

As often is the case with Classic Mac Systems, sometimes really odd messages can be the result of other things gone awry, so may I also add trying a few tried and tested troubleshooting tips along with the good 'ol  remove extensions trick

1. restart your Mac with your extensions off; this one should be done before you start removing your extensions because it quickly can point to the culprit, aka, where your issue is or is not, then try opening your chooser - if it still bombs, then this should tell you that your issue likely lies elsewhere

2. remove your preferences, or their folder in System 7 and higher, and restart your Mac, or if you're lucky, just open your resource or app (in this case the chooser) again, if this works, then you can hunt for the preference that needs to be recreated and at that point, hopefully that'll be the end of that issue - disclaimer: my experience with this particular fix comes mostly from System 7 and higher

3. zap your pram by holding down your your Command, Option, P and R keys together when you restart your mac and hold until you hear your Mac's startup sound (on a Mac emulator like Basilisk II I believe you never hear the startup sound, but like anything else in life, I've learned to never say never lol)

there are other tricks like these but I like these three for their simplicity and ability to rule them out in the case of a bigger or simply more obscure issue clogging up the works
